Position Overview:
We are seeking a caring and dedicated Hospice CNA to join our team. As a Hospice CNA, you will provide direct patient care, assisting with activities of daily living and ensuring the comfort and well-being of our patients in their homes. Your compassionate approach will be vital in enhancing the quality of life for individuals at the end of their journey.

Responsibilities:
- Provide personal care to patients, including assistance with feeding, bathing, grooming, and toileting as part of your role as a Hospice CNA.
- Assist with patient mobility, transferring, and positioning to ensure comfort and safety.
- Take and record vital signs, including temperature, pulse, and blood pressure.
- Observe and report any changes in the patient’s condition to the Hospice team.
- Help with medication reminders and provide emotional support to patients and families.
- Ensure the patient’s environment is clean, safe, and comfortable.
- Document all care provided in compliance with company policies and HIPAA standards.
- Travel to patient homes, providing consistent care within the service area.
Qualifications:
- A minimum of 1 year experience as a hospice CNA is required.
- Valid Illinois CNA certification is required.
- CPR and BLS certifications are required.
- A valid Illinois driver’s license, auto insurance, reliable transportation, and the ability to travel within the service area are required.
- Compassionate, empathetic, and dedicated to providing the highest quality of care.
- Ability to work independently and as part of a multidisciplinary team.
- Strong communication skills and the ability to build rapport with patients and families.
